Job Description:

• $105-160k based on experience
• Mon-Fri full time, day hours
• Permanent position with excellent benefits including qualification for the NHSC Loan Repayment Program
• Will deliver comprehensive quality patient care, including diagnostic and therapeutic interventions according to standards of practice to children and adolescentsKey Responsibilities & Skills
  • Provide comprehensive primary care to pediatric patients, including preventive care, chronic disease management, and acute illness treatment.
  • Utilize evidence-based clinical guidelines to ensure high-quality, family-centered care.
  • Work collaboratively in a team-based model, engaging with physicians, nurses, behavioral health providers, and support staff.
  • Apply excellent clinical judgment and demonstrate an empathetic bedside manner with children and families.
  • Educate parents and caregivers on pediatric health concerns, growth and development milestones, and preventive care.
  • Maintain thorough and accurate documentation within an electronic health record (EHR) system.
  • Exhibit a strong understanding of population health concepts and proactive care strategies within a PCMH team.
  • Demonstrate proficiency in patient education principles, supporting families in developing effective self-care plans.
  • Maintain confidentiality and uphold professionalism in all interactions.
  • Adapt to a fast-paced medical environment with changing priorities and responsibilities.
  • Participate in community outreach and health promotion efforts as needed.
  • Travel between clinical sites as required.Qualifications:
    • CT Nurse Practitioner license. Pediatric Nurse Practitioner preferred, Family Nurse Practitioner can be considered
    • Prior work in an outpatient setting and/or with socioeconomically diverse pediatric populations a plus
