Job Description:
A company is looking for a Behavioral Health Care Manager - LMSW
/ LISW-CP (REMOTE).
Key Responsibilities
Provide behavioral health care management services to enrolled
members
Conduct outreach calls and assist in assessing members' needs for
care management services
Document all care management activities in the electronic care
management documentation system
Required Qualifications, Training, and Education
Bachelor's Degree in Social Work, Nursing, or Human Services from
an accredited institution
Current valid, unrestricted state license in a behavioral health
specialty or nursing
Three to five years of prior experience in behavioral health or
human services
Certification in Case Management (CCM) or eligibility to take the
exam within 24 months of hire
Experience in customer service and office settings preferred
