Job Description:
A company is looking for a Senior Health Navigator, Monitoring
to provide second level monitoring of medical readings and assess
patient needs.
Key Responsibilities
Provide second level monitoring of medical alert alarms and contact
patients to assess their needs
Support triaging and assessing data represented by alerts to
determine next steps
Assist with clinical enrollment of new clients and maintain records
of contacts and orders
Required Qualifications and Education
High School Diploma required
One (1) plus years of experience in contact center or customer
service with heavy phone volume
LPN certification is preferred
Exceptional customer service and people skills
Ability to work flexible hours and overtime when needed
