About This RoleCareConnect is seeking a compassionate and dedicated Home Health Aide to provide exceptional care to our clients in their homes. As a Home Health Aide with our team, you will play a vital role in helping our clients maintain their independence and quality of life.Responsibilities

• Personal Care and Assistance - Provide personal care to patients, including bathing, dressing, and grooming, to ensure their comfort and dignity.
• Assistance with Mobility and Transfers - Assist patients with mobility and transfers to ensure their safety and well-being.
• Medication Administration - Administer medications as directed by healthcare professionals to maintain our patients' health and well-being.
• Vital Sign Monitoring - Monitor patients' vital signs and report any changes or concerns to the appropriate medical personnel.
• Emotional Support and Companionship - Provide emotional support and companionship to patients and their families to enhance their overall well-being.
• Environment Maintenance - Maintain a clean and safe environment for patients to ensure their health and safety.Requirements
• Certification - Be certified as a Personal Care Assistant (PCA) or Certified Home Health Aide (HHA).
• Understanding of HIPAA Regulations - Understand and adhere to HIPAA regulations to maintain patient confidentiality.
• Cooking and Light Housekeeping - Be able to cook simple meals and perform light housekeeping duties as needed.
• Training and Skills - Have satisfactory completion of a New York State Health Department-approved HHA Training Program or equivalency and possess adequate literacy and simple arithmetic skills.
