Job Description:
A company is looking for a Care Management Associate to support
healthcare coordination through telephonic outreach and member
engagement.
Key Responsibilities
Conduct telephonic outreach and enrollment of eligible members into
care management
Facilitate case assignment and connect members to care managers
through warm transfers
Maintain accurate documentation and adhere to compliance and
confidentiality standards
Required Qualifications
Must reside in Oklahoma
High School diploma, G.E.D., or equivalent required
2 - 4 years of experience in the healthcare field preferred
Ability to manage multiple priorities and adapt to change
Experience with Microsoft Office applications preferred
